The Housing First Service primarily aims to permanently house individuals with chronic addictions and provides comprehensive intensive support to ensure these individuals can sustain their own tenancies within the community through a case management approach.
Case management is a process of intense engagement with a service user with a broad range of complex needs (e.g. addiction, mental health, physical health or a combination of all) with intensive support requirements.
Case management works to ensure that multiple services are effectively coordinated to respond to the needs of the service user.
The case management process is led by the support worker, who coordinates and manages the service users support plan which is service user centric.
Qualifications / Experience:
3 GCSEs A-C to include Maths and English or equivalent.
1 year’s paid experience or 2 years’ volunteering experience working with vulnerable adults with complex needs.
Experience of multi-agency working.
Good working knowledge of risk assessment and risk management processes.
Working knowledge of using IT systems.
Good written and verbal communication skills.
Good IT Skills.
The ability to undertake outreach/lone working.
Ability to manage a case load time.
A knowledge of the issues around supporting individuals with substance user issues and complex needs with problems in non-residential and non-specialist settings.
A knowledge of Harm Reduction methods.
A good understanding of issues presented through alcohol and substance use.
Ability to liaise with external agencies.
Ability to work on a rota basis (7 day week Mornings, Evenings & Weekends).
A full driving licence and access to a car insured for business purposes.
